It is an early style craft/utility knife made by Gillette.I have a few of them along with a couple packages of replacement blades.To change blades,push down on the bottom lock(just like a modern day lockback)and pull the blade forward.If it has been in there a long time,put a couple drops of penetrating oil around the tang of the blade and use a pair of pliers to pull it from the handle.Or use a piece of duct tape over the edge of the blade to prevent slicing and cursing.

Replacement utility blades are still available from X-acto, and (are you ready for this?) surgical blades from Steri-Sharp would also fit.
