Repair form questions

I am receiving a lot of knives from viewers of my channel, and I have started having them fill out a form. It started as a way to keep track of which knife belonged to which person, then I added a bit for legal liability.
I was wondering if anyone else has something similar? Any suggestions? I do not want too much legaleze, but I do want to manage people's expectations as well.
Here is a copy of the text:

Some handle materials and certain replacement parts may be more expensive due to the price and availability of materials. Pricing will be provided before the work begins. By signing below you understand the following:
1) Full payment is due after the work had been completed.
2) The owner assumes responsibility for the finished product and any changes in value to the knife. I do my best, but there may be flaws in the finished work.
3) No work to be performed will in any way be constituted as counterfeiting. You knife may be unobtrusively marked as having been repaired.
4) Pictures and/or videos of the knife may be used and posted online. The owner may opt to NOT have pictures or videos posted by initialing here: __________
5) Due to privacy, the owner’s full name and other identifying information will NOT be shared online. If the owner wants any information such as first name, nickname, city or state to be used online, write that information here: _____________________________________________

You might want to include a spot for the customer's email.
I do all of my business via email-start to finish. That way I have records of the conversation from start until I send a receipt for payment received.

I set all of mine up in email, but you are correct that it would work much better if I added it to the form.
