It was gone (sold) when I first looked. I have a copy, but if a guy has hundreds of knives, an extra copy of that book only seems right!

Well, that one sold for $46.00 and there were multiple bidders, so I suspect it achieved fair market value. I don't recall what they were when new, but they were not cheap. They were self-published through Kinkos or possibly at home and the numbers were limited.
He has said there will be no more revisions or re-issues.
